How to do it
- Upload the visit recording. Drop the audio file of the visit directly into AI. Paste in the blank note template you use in your charts. Tell AI to match your typical format.
- Let AI draft the note. AI listens through the visit audio and sorts the conversation. It drafts the chief complaint, history of present illness, exam notes, and proposed plan into your template. It leaves unknown details blank.
- Review against your memory. Read the draft on your screen with the patient file open. Correct any missed medications, fill in your physical findings, and update your clinical plan. You hold the final say on the chart.
- Paste into your chart. Copy the reviewed text over to your electronic health record. Sign the note and close out the encounter for the day. You move on to the next patient without evening backlogs.
What to type first
I attached the audio from today's patient visit and my usual clinic note template. Transcribe the visit and organize it into this exact format. List the symptoms, history, physical exam remarks, and discussed plan. Do not invent any findings, do not assume physical exam details not spoken aloud, and do not send anything outside this chat.
Change the words to match your business. AI follows what you say, so say what done looks like.
Before you trust it
- Check all listed medications, dosages, and allergy mentions against what the patient said.
- Confirm the physical exam section only contains observations you spoke during the visit.
- Make sure the assessment and plan reflect your own clinical judgment.
Once visit notes feel routine, you can have AI draft the patient's after-visit care instructions using the same visit recording.
