Turn patient visits into notes with AI, for doctors

How to do it
- Paste your template and examples. Put your preferred note layout into the chat window. Add a couple of past notes so AI sees how you like to phrase plans and findings. This shows AI how you write.
- Record your raw visit summary. Dictate what happened as soon as the patient walks out. Speak naturally about the symptoms, exam findings, and your plan. Do not worry about clean grammar while speaking.
- Ask AI to organize the text. Send the spoken text into the chat with your prompt. AI sorts your words into the correct sections of your template. It leaves out filler words and organizes the draft.
- Read and fix the draft. Review the clinical facts, medications, and next steps line by line. Fix any medical phrasing that does not match your judgment. Copy the final text into your patient chart.
What to type first
Here is my clinic note template, an example of my style, and rough dictation from a patient visit. Draft a clinical visit note using only the details from my dictation. Put the facts into the template sections. Do not invent symptoms, dosages, or exam findings. Do not send or save anything outside this window.
Change the words to match your business. AI follows what you say, so say what done looks like.
Before you trust it
- Check all medication names, dosages, and frequencies against what you prescribed.
- Verify that the physical exam section lists only the maneuvers you actually dictated.
- Look at the plan to make sure follow-up timeframes match your medical intent.
Once drafting notes feels natural, try having AI turn your final visit plan into simple discharge instructions for the patient to take home.
