How to do it
- Remove patient names and identifiers. Strip names, dates of birth, and record numbers from the pages. Paste only the medical history, test findings, and treatment notes into the chat window. Keep the real file open on your other screen.
- Paste the history into AI. Send the notes along with your outline request. AI reads the records and groups chronic conditions, past surgeries, active medications, and recent lab trends together. A draft appears in your window.
- Compare the draft against records. Read the draft against the original reports on your screen. Look closely at medication dosages, abnormal lab values, and dates of prior procedures. Fix any missing context or wrong details directly in your note.
- Save to the chart. Copy the corrected draft into your clinic record system. Add your own clinical thoughts and sign off before you walk into the exam room. The visit starts with the full story in front of you.
